At Rebalance Community Care, we believe disability support should be led by genuine clinical expertise — not just good intentions. As an NDIS provider based in Brisbane and surrounding areas, we support participants across all disability types, with particular experience in complex and clinical needs that can be difficult to place with a standard provider.

Our approach is trauma-informed and person-centred from the ground up. Every support plan starts with what matters to you, your goals, your needs, your pace, backed by clinical governance from a Nurse Practitioner and a director with over 30 years' frontline healthcare experience as a paramedic and former registered nurse.
